IGBO 300 Game / 800 Series Award Application Form

Please provide the following information to claim either a 300 Game or 800 Series IGBO Award.   If the bowoler completed an 800 and a 300 award in the same series you can select that as well; however, the same shirt color, style, and size will be applied to both.   If the bowler wants two different types of shirts then PLEASE subject two separate request.   Upon submission your form information will be sent to our awards administration team for review and processing

Award Preference

In accordance with the IGBO award program she/he is entitled to one 300 and one 800 award per fiscal year (July 1 to June 30).   Additional awards may be purchased for multiple honor scores bowled during a fiscal year.   Contact Rob Silliman for additional orders.
